You are members of the behavioral health team, charged with developing a biopsychosocial evaluation. This should not be a treatment plan, but an assessment of the person’s current problems and level of functioning based on the information provided.Personal Information: Sam is a 63-year old Asian American male who lives with a male roommate in an urban area. Sam has been divorced for 15 years. He has two adult children, who are both married.Current Problems:Sam has been drinking alcohol heavily for more than 30 years. He is a daily drinker. He estimates that he drinks at least 1 liter of vodka each day; while his pattern of drinking used to be confined to the late afternoon and evenings, it has spread to “pretty much the whole day.” He claims that alcohol is the only thing that makes him feel relaxed. He is also a heavy cigarette smoker and estimates that he smokes one to two packs per day. Medical problems include mild chronic obstructive pulmonary disease, frequent stomach upsets, and mild insomnia.Psychosocial Data:Sam was born in Vietnam, where he lived until he was 20, when he immigrated with his wife to the United States. He described his upbringing in Vietnam as “hard,” and stated that both of his parents were farm laborers. They are deceased and his father was an alcoholic. After arriving in the U.S., he attended night school to attain his GED at age 29. In the past 30 years, he has worked sporadically as a clerk in a convenience store. He lost his job 2 years ago due to absenteeism. At this point, he spends up to 12 hours a day sleeping in his apartment. He has no contact with any family members and claims he is not close to his roommate.Appearance and Behavior:Sam is a short, slender man who is not neatly dressed. He walks slowly and appears to be angry based on his facial expressions and other nonverbal behavior. He looks older than his stated age. During the interview, Sam spoke slowly and deliberately with a strong accent. His mental status appears normal, but there are marked problems with short-term memory and word-finding.Psychological Profile:Sam has never been evaluated or treated by a mental health professional. He reveals that “life is not so good” for him, and if he does not drink, he feels worse. His view of the world and himself is pessimistic.
