The effect of provider job
satisfaction and burnout when a non-PCMH site becomes PCMH

Background:Patient centered medical home (PCMH) sites have been
on a steady rise over the past few years.
It is being embraced by many, in hopes that it will improve healthcare
in the United States by reducing costs and improving patient outcomes. .
However, as important as cost and patient outcomes are, another integral part
of the PCMH equation isn’t discussed very often: the providers. How will PCMHs
impact providers? This systematic review of literature will investigatepractice
change from non-PCMH to PCMH, and its effect onthe satisfaction and burnout of
providers, using a wide lens and multiple perspectives to understand both the
details and the overall success of the transformative change process.

Methods:A complete search of current medical literature will be performed, this
will include, but not limited to: UC Health Science Library, Medline-OVID,
EBMR-Cochrane Review, and CINAHL, using the keyword patient-centered medical
home and the MESH terms patient centered care, PCMH, non-PCMH to PCMH, job
satisfaction, burnout, professional, provider, provider satisfaction, provider burnout.
This systematic review of literature will investigate the use of PCMHs and its
effect on the satisfaction and burnout of providers.Because we want to
understand the mechanisms as well as results of practice change towards a PCMH,
we will be collecting quantitative data in 3 key points, namely; 1-Intermediate
process measures (staff perceptions about the providers and the organization),
2- Patient outcomes (patient ratings of their experience with the practice,
providers and measures of care quality), 3-Provider satisfaction and burnout
(providers are overwhelmed, contented or dissatisfied). Evidence-based
materials and literature will be used to highlight the benefits of a holistic
view of the practice change process, outcomes and impact on providers.
